Collaborative, multidisciplinary care team
Provide primary and preventive care to adult patients
Diagnose, treat, and manage chronic and acute conditions
Develop individualized treatment plans and coordinate referrals
Collaborate with behavioral health, dental, and care management teams
Maintain accurate clinical documentation and uphold quality standards
MD or DO, Board Certified/Eligible in Internal Medicine
Active CT medical license, DEA, BLS, and ACLS required
Community health or primary care experience preferred
